dc.description.abstractThis paper helps improve patient outcomes due to the enhanced information sharing, coordination, and establishment of comprehensive care plans. Integrating electronic health records (EHRs) into the interprofessional collaboration care model facilitates coordination, collaboration, and shared decision-making, leading to high-value care and improving health outcomes.
